Below is a copy of the Annual Business Registration Form (ABR) required by the Authority’s Terminal Tariffs. This must be completed by ALL firms that conduct business with the Authority or on Authority premises. All Tariffs are available online at www.port-of-charleston.com. As stated on the ABR form, it is your responsibility to read and understand Tariff provisions, particularly the General Rules and Regulations portion (Tariff No. 8, pages 1 through 10B) (Tariff No. 21, pages 1 through 14A) (Tariff No. 2, pages 1 through 13A). If you require a hard copy of either Tariff, please contact Ms. Dale Morris at 843-577-8176.

Please complete the ABR Form in its entirety for Authority Fiscal Year 2009. Return ABR Form to the attention of the Risk Manager no later than June 30, 2008..

NOTE: Certificate of Insurance is required. Certificate holder shall be SC State Ports
Authority, Attn: Risk Management, PO Box 22287, Charleston, SC 29413-2287 (Fax No.
843-577-8138).

All Commercial General Liability (CGL) Policies must be endorsed indicating the South
Carolina State Ports Authority as an ADDITIONAL INSURED. The CGL and other Commercial Liability Policies shall be amended by endorsement to waive the insurer’s right of subrogation against the SCSPA. Failure to complete the Form and provide the required insurance certificate(s) with endorsements may result in being denied use of port facilities.

IF YOU NO LONGER REQUIRE ACCESS TO SCSPA FACILITIES, PLEASE NOTIFY US AT 843-577-8176 OR RETURN THE ABR FORM WITH YOUR COMPANY NAME AND N/A; ALSO
PLEASE ADVISE YOUR INSURANCE AGENT TO DELETE US FROM YOUR CERTIFICATE
HOLDER LIST.

All vehicles utilized on terminal by your company personnel must be properly placarded,
